Cory Monteith’s final coroner’s report says the actor took heroin and drank alcohol on the night he died.

According to the report, his official cause of death was "mixed drug toxicity, involving intravenous heroin use combined with the ingestion of alcohol."

Responders who arrived on the scene noted the discovery of a "spoon with drug residue and a used hypodermic needle." They also found two empty champagne bottles.

The report notes the 31-year-old Glee star "had a history of illicit drug use with intermittent periods of rehabilitation and of abstinence from drugs."

The coroner believes Cory's tolerance for drugs had dropped thanks to him staying sober since going to rehab in April.

The actor is being honoured on Glee, which just started airing its new season on Fox network in the United States. The third episode will centre on the death of Finn Hudson, Cory’s character on the show. While many details have not been released about the tribute, E! Online reports it will take place in Lima, Ohio and be about Finn’s death. The characters on Glee will then remember and mourn Finn, whose cause of death will not be revealed. Cory’s real-life girlfriend Lea Michele appears in the episode but not until the end.

"We've mourned Cory, but now we're mourning Finn. So it's kind of like two people in that one person,” previously Matthew Morrison told E! News. “It's incredibly hard, and we're staying strong together, and I have to say, Lea [Michele] has been the most strongest of all of us."
